Manly Waters Private Hospital has a strong tradition as a GP hospital and welcomes applications for Visiting Practitioners. For further information please complete the steps below. 

Step 1: Please download the following form: Manly Waters Visiting Practitioner Rights Application then upload to our online Specialist Accreditation Application portal below.

Step 2: Please prepare the following documentation for upload

• Current Curriculum Vitae
• Photocopy of current AHPRA registration
• Two written references
• Hand Hygiene Certificate
• Working with Children Check
• Copy of Radiation License – if you intend to operate x-ray equipment.
• Photocopy of current professional indemnity insurance certificate of currency showing:
  – Period of Insurance
  – Amount of Cover
  – Category of Insurance

  Step 3: Complete the Specialist Accreditation Online Form
